AS a guess, it looks like a igelan error. I would make sure you have the patches PHNE_34427 and PHNE_33622 or the more recent PHNE_35692 (and any pre-requisites or co-requisites).

The panic seems to be caused at streams layer. Any recent changes on the system? Patches or TOUR bundle installation?
