scsi error
I am getting the above error in syslog file , any clues whats wrong

Search the forum... you will find lots of hints. Most of them will deal with hardware problems (controller, cable, termination, device...)

Run ioscan -kfnC ctl and find the SCSI ciontroller with ID=4. The problem is with it.
If you connected something new to this SCSI, check SCSI ID duplication or terminators.
Look at output of
ioscan -fn
for all devices connected to this SCSI and check their physical connections.

This will load you a graphical interface there you can find in LOG tools detailed informations of problematic device.
This error can indicate problems.
My experience is that you will have to replace disk. We did it many times after this message in syslog.
Check cables and termination of SCSI device.
