Seems to be a problem whith some of our disks. Very often there is a proccess very heavy in cpu. It's lvmkd.

Based on the message, you have a disk that has either failed or is very close to failure.
I'd plan on a replacement, preceded by a backup if the data is still accessible. It seems to involve only one disk.

maybe one disk has already crashed!

Solutionyes, SEP is right and you should soon work on that disk. Anyway they seem two disks involved:

Also I already faced more times that message
Line 5102 in /ux/core/kern/common/io/pat_psm.c: pat_get_olar_info - rc -1 slot_id -16187515
and it is a sympthom that you should update or GSP or PDC firmware revision.

the faulty disk in question should be c0t8d0.
1f008000
1f=major number for sdisk driver.
00=instance number for this controller

Run some tests on the disk, such as diskinfo and dd. If you have diagnostics installed there are tests to exercise the disk.
pat_get_olar_info is an unrelated message that indicates you have a Lclass or Nclass, etc. that is running older firmware for GSP and PDC. You may want to look at updating those.

Try to run this command,
# dd if=/dev/rdsk/c0t8d0 of=/dev/null bs=64K count=1000
and see if this is successful.

Are they mirrored between them? Are they belong to the same VG? The good suggestion is to launch a dd to check and consider to open as soon as possible an HW call by HP. What about PE on these disks? You can see that by lvdisplay on logical volumes belong to them.

If the disk is truly gone these tools won't see it. Your ouput indicates they will fail an stm disk excerize.
