I recently upgraded to Sendmail 8.13.3. The only concern is to make sure that you have modified /etc/sendmail.cf to match your current configuration. I would suggest making a copy of the existing sendmail.cf file then upgrade.
Diff the new sendmail.cf and the old sendmail.cf and apply the changes.
It is better to do that then keep the old sendmail.cf file.

details like whether you start with a patch

It's not really all that hard.
Back up all the configuration files in /etc/mail and then swinstall the depot from HP http://software.hp.com
You back up the configuration files merely in case something gets trashed. The upgrade deals with several major security faults, so there may be conflict with any customizations you have made to the configuration files.
