I can see in syslog of my 11.31 ia system the following vmunix messages:
vmunix: read_efi_hdr: physio failed for dev_t d000093
vmunix: read_efi_hdr: physio failed for dev_t d000089
vmunix: read_efi_hdr: physio failed for dev_t d00008a
vmunix: read_efi_hdr: physio failed for dev_t d00008b
vmunix: read_efi_hdr: physio failed for dev_t d000093
vmunix: read_efi_hdr: physio failed for dev_t d00008c
vmunix: read_efi_hdr: physio failed for dev_t d000089
vmunix: read_efi_hdr: physio failed for dev_t d00008d

Any ideas why this can happen?

The last login on the system was more than 12 hours before the vmunix message.
The probability of a ioscan -e is almost null, I believe.

SolutionPlease check this PHCO_40961 patch description:

Then go by all QXCR1000913961 tab's ocurrences.
As you see the vxconfigd(1M) is involved here. The Volume Manager configuration daemon, vxconfigd, is responsible for maintaining configurations of disks and disk groups in the VERITAS Volume Manager. vxconfigd takes requests from other utilities for configuration changes, and communicates those changes to the kernel and modifies configuration information stored on disk. vxconfigd is also responsible for initializing the Volume Manager when the system is booted.
#man vxconfigd (see syslog ibrary calls og all of its regular console messages).
