unable to mirror vg00

I am working on an 9000/800/N4000-44 running HP-UX 11.00. When I try and mirror vg00 it corrupts when I get to lvol3. I am able to mirror lvol1 and lvol2 but when I try lvol3 I get inode errors in the syslog. If I reduce the mirror to 0 the inode errors stop but the box has a lot of corruption. I am unable to unmout files, ioscan causes core dump, the major number for /dev/vg00/group is zero, a directory called /dev/snap is created and has a copy of /dev....
root /etc> vgchange -a y /dev/vg00
Couldn't query volume group "/dev/vg00": Invalid argument

I have replaced both internal drives, ignited, and get the same errors when I try and mirror.
I have June 2001 patch bundles.
HELP....

In your syslog excerpt I see some entries for /dev/root. I could see where you'd run into problems if / were mounted as /dev/root instead of /. Do a 'bdf' and check. If is is /dev/root do the following:

# mv /etc/mnttab /etc/mnttab.bad
# mount -a
# bdf

Check and see if it is now mounted correctly.

Also check to make sure that MirrorDisk is in the configured state and not just installed.

# swlist -l fileset -a state | grep -i mirror

You did install the correct version of Mirror Disk for this machine? I would also see about getting the latest MirrorDisk patches. There could be some known issues that are patched. You could try going through the custom patch manager to see what patches it things you need on your box.

Can you also post the steps you were taking, just so we can have a look and make sure nothing looks funny in your process?

Your syslog mentions "/dev/root". I usually see this when someone has gone directly to multi-user mode from lvm maintenance mode. If you do a bdf and / shows up as /dev/root instead of /dev/vg00 this is likely your problem. If so:
Move /etc/mnttab out of the way.
Do a "mount -a".
Try miroring again.

I'd like to see the full vgdisplay output and whatever ioscan tells you. I see a few references to EMC. If the host is attached to a Symmetrix, would you tell us what is the configuration (eg what the EMC inq command spits out)? If the host is using PowerPath, I'd like to see the output of `powermt display`.

Can I assume by the silence that you guys are stumped by this question?

Hi Justin,

I presume the /dev/root issue is taken care of, right?

When you Ignited, did you re-build from a backup of this server? Did the problem occur before or after rebuilding?

Are the 2 internal disks the only ones in vg00?

I saw lvol4 filled. Unless you edited the syslog you posted, it appears you didn't boot before posting. Have you rebooted since?

Is it time to "drop back and punt" - rebuild the system from scratch?
