Also the problem is happening on our another two production system. Same as apu002, they are 11.11 HPUX.

"insf -e" will NOT require a reboot.
Well, if you have problem in rebooting the server and wants something like binding the correct driver to your FC, try:
#ioscan -M
Things can get clear if you can post output of:
#ioscan -fnC fc

How did you install the driver bundle ? The installation instructions for FibrChanl-00 bundle state that reboot after the installation is necessary. I suppose that this is due to the fact that fc driver is statically bound to the kernel.
I would suggest to try to reboot the system. If this does not help correcting your problem, reinstall the driver bundle according to the installation procedure.
