While taking ignite backup below error is giving.

check for HW failure ... EMS messages in syslog ...
diskinfo on the device file.

Probably, this disk might have removed from SAN.
If you can't find this disk in your ioscan output, make a backup of your lvmtab and do vgscan to create new lvmtab file.
Also, verify, your lvlnboot -v command displays without any issue.
If you can't see this disk, since other pvlinks available, need to remove the device file using rmsf command.
Make sure, the disk availability.
