Hy,
I was checking with vparefiutil -M, the hardware paths when i found out, something strange, I think it had and explanation, but I am bit slow this morning, just can't remember..
my boot/monitor disk is not listed in the ioscan/olrad output:
[root@rx:~]# setboot
Primary bootpath : 1/0/0/3/0.6.0
HA Alternate bootpath :
Alternate bootpath : 0/0/2/1/0/4/0.5.142.0.0.0.0
as you can see I boot from internal disk 1/0/0/3/0.6.0 which is where from the vpar monitor loads, but the HDW path is no where to be found, I only see HDW from cell 0:

Where exactly is your problem?
The internal disk is connected to a built-in controller.
"(ol)rad -q" lists only hot-swappable slots and controllers, the built-in is "fixed" and not listed.

I couldn't find the hdw path 1/0/0/3/0.6.0 on the vparefiutil -M, nor on the ioscan.
God knows what I was greping, it's all there know :o.
