1) The fact that windows saw the disk only means the disk is not failed. Beyond that this fact is irrelevant.
2) You should check the fiber cables and wiring of the MSA-1500 to insure that lights are lit and the configuration is normal. Item 1 does not definitively prove anything as the wiring could have been changed or damaged.
3) fdisk -l # lets see the output
4) lspci Lets see what kind of cards you have. I'm guessing qlogic. You should make sure they are certified with the OS and hardware.
5) Try a different version of cpqacuxe. I've seen that RH 4 update 5 is certified but you need a version of the utilities that is certified with the OS and the hardware. We ran into this same problem with RH 4 update 4 and a bad version of the disk utilities.
6. uname -a # I'd like to know if we're talking 32 bit or 64 bit OS.

Do you really have an emulex hba and a qlogic hba in the same machine at the same time?

I've neer seen a setup like that before. That may be a problem under Linux. Is it possible to let me know which one is plugged into the SAN? Qlogic in general is built into the kernel and auto detected. If your SAN connection is via emulex, you need drivers to isntall before you can see the disk.
I'm leaning toward a bad version of cpqacuxe. Please try another version.

I have dl360 g5 /RHEL 4 U5 (x86-64)
According to this link the recommended version of acu is 7.80.6, which is also what I have.

the array is attached through the qlogic HBA.

I just wanted to let you know that the problem has been fixed "Edited to comply with ITRC guidelines"
it was related to an incorrect zoning on the FC switch. More details follow. Thanks again for your help.

SAN Zoning
1.) http://16.58.249.223/switchExplorer.html --> Web Interface of the Brocade SAN-Switch
2.) Zone Administration
- HBA WWN of first DL360
- HBA WWN of second DL360
- MSA port
--> all into the some SAN zone
(by default: there is no zoning on the SAN Switch, all WWN are in the same zone)
Make LUN visible to both DL360
1.) https://16.58.250.66:2381/ --> click on HP Array Configuration Utility (ACU) Link
2.) ACU Web Interface --> MSA Controller --> Selective Storage Presentation --> avtivate --> mark LUN as visibel to both HBA WWN --> save all settings
Control on the Server
type:
hp_rescan -a
lssd (you should see the device file --> sda & the MSA)
