HP P400 Array card.    configured raid 5 (three 146GB SAS Hard Drive)

DL580 G5 is not supported on Server 2012.

If you boot to an OS CD then none of the drivers I tried for P400 will work.

However if it already has Server 2008R2 which is supported you can run an upgrade install from within the Server 2008 R2 GUI and it will pick up the P400 drivers and upgrade to Server 2012.

Which implies that the Server 2008 R2 drivers will work for the P400. I would copy them from the Server 2008 R2 installation and present them during install of Server 2012.
