It should stand for "Internals and Data Structures". The quote is also present on page 3 of "VAX/VMS Internals and Data Structures Version 4.4", Ruth Goldenberg, et al.

Thanks go to Ruth Goldenberg for that quote. I always figured that she used it as a metaphor for the design of VMS, specifically in relation to the processor modes.
