EVM Log Error 500 - Connectivity has been lost for device (HWID=255 lid=36 btl=0/0/0)

I get this error few times, can anyone help me fix this.

Can you please post the output of:

hwmgr v d
hwmgr show scsi
Por que hacerlo dificil si es posible hacerlo facil? - Why do it the hard way, when you can do it the easy way?
