How to add the details in table format?

HI All,

We are using MQC v 11.0.0.724.

We have a requirement to fill the info in table format in defect.

Let say I have XX_tab in defect and if I open this tab while creating a defect  it should show the information in the format which is attached.

let me know if more information needed.
