I've update the firmware for the smart array v5.06

Error is always pointing to the same disk as the source which leads me to believe its the HD. Looking for a few opinions and hoping it isnt the controller. Any suggestions or advice is much appreciated

Hi, I'd load up HP ACU and look in there. Any errors/messages?

If you want to check even deeper, fire up 'HP ADU' and get some detailed diagnostics for the array controller and the disks.
