I have a proliant ML350 G4 and CPU0 is constantly running at 69C. CPU1 is constantly running at 45C. Both fans are working properly but from time to time CPU0 will spike up causing a shutdown response. I do not want to change the environment response.
Aside from affinity settings - any ideas to why on CPU is always running hotter than the other?

CPU temperature always different between CPU's on same server.

For example, I checked now CPU temp on our ML370 G4:
CPU0 current temp 31 temp threshold 57
CPU0 current temp 40 temp threshold 80
So, your real problem is not why "one CPU is always running hotter than the other", but why "CPU0 will spike up causing a shutdown response".

I have 5 proliant servers
all are dual processor and all have had the board replaced.
3 of the servers show the disparity in CPU temp
However 2 of them report running at exactly the same temp 29C
The shut down occurs on one server because of poor temp conditions when a door is shut (ughhhh - out of my control).
The thought process here was; if I could get the CPU's to run equally at a lower tempurature there would be more leadway between operating tempurature and the threshhold.

Is there room in the room (and the budget) to put an AC unit - they make some decent in-room units (look at Bed&Bath, Lowes, etc.). Just run a felxible vent hose off the exhaust.
With little space or $$, try cutting an oening at the bottom of the door and installing a grate or louver, and a similar opening at the top of the door with a blower to move the hot air out of the room. The displaced hot air will pull in the cooler air from the floor.
