proliant g6 driver detected a controller error on \Device\Harddisk0\DR0

Had a problem where user wasn't able to access their data but server was still up and running. Event viewer for server 2008 showed event id 11 "driver detected a controller error on \Device\Harddisk0\DR0"

restarted the server and the drive in bay 5 had been disablled, reenabled this and booted server. everything seems to have come up fine, no data missing and back to normal functionality.

this is the 2nd time in last 3-4 months that this has happened. firmware update was done inside last 12 months, this is my next port of call once school term finished in a weeks time.

attached is report from HP array diagnostics utility, can anyone pinpoint what might be causing this issue?

drive 5 shows Last Failure Reason Hot Removed (0x14)

Hi,

According to ADU report, the firmware on the P410i is very old, the current firmware level available at HP is 6.00 (B).

Smart Array P410i in Embedded Slot : Internal Drive Cage at Port 1I : Box 1 : Drive Cage on Port 1I : Physical Drive (72 GB SAS) 1I:1:1 : Monitor and Performance Statistics (Since Factory)
Read Errors Hard                     0x00000001

Refer to this doc.
