Smart Array P840 Error reported in ILO, but fine otherwise

I have an odd situation.

The ILO management page for my ML110 Gen 9 server is telling me that the P840 card has a problem, it lists "Controller Status: ERROR". I have receieved some intermittent SNMP 11020 traps messages, the source is "Foundation Agents", the description is "A 'Host Health status change' trap indicates that health status has changed." and under details it states "Change in health status 'FAILED'".  Just before reciving these SNMP mesaages I get an additional SNMP message that is identical, but under details it states "Change in health status 'UNKNOWN".

The server status light is still green, indicting nothing is wrong with the server hardware. The HP smart sotrage administrator lists no issues and I can see all of the card infomration and arrays. All drives an arrays are working as expected. I performed a reboot to see if anything came up during POST and all messages came up clean. 

Any idea what is going on?

Make sure that iLO4 firmware is upgraded latest version.
