NetServer LC3 and SureStore DAT24

NetServer LC3+Adaptec AHA2940+SusreStore DAt24, OS: Netware 5.1

The problem: the drive dies when I insert a tape (cleaning or normal). The LEDs are off, the engine doesn't spin up and the most worrying: the only way to eject a tape is to power cycle the server.
Has anyone had similar problem? What could be the cause and how to fix that?
