Menu Items Missing & Unable to drill down by clicking on a server in a list

Hi everyone:
Yesterday I was suddenly made aware of a problem that our SIM implementation has come up with. Everyone has lost the ability to click on a server name in a list to drill down into the server properties page. In addition, when I logged in, I noticed that the menu choices have changed. My menu is missing the CONFIGURE, DIAGNOSE, and OPTIMIZE items. Under each menu item that exists, many items are missing. For example, under Tools, there is only Custom Commands menu item. The System information -> System Page menu is gone (the same thing that clicking on a host brings up!
First I thought that someone stripped away rights unknowingly, but all looks normal, and my account is still administrator, with authorizations for all tools and all managed systems. Additionally, I can create/delete users still. So I did, and created a new administrative user to test with, and no help.
After searching around on here, there was a couple suggestions pointing to resources, so I first restarted the service, but that didn't help. Last night I had the system rebooted to clear any cobwebs out of memory, and that didn't help either.
I'm a bit at a loss on how to fix this. Anyone have any suggestions/ideas?

We had occasional issues like this reported about HP SIM 4.0 but not really with 4.1. The common cause was typically a space where it wasn't supposed to be, usually associated with a blade enclosure. This would screw up the ability of JavaScript to parse through lists. 4.1 did a data integrity check to eliminate it.

Are you using 4.0 or 4.1?

Please note if you stayed on 4.0 because 4.1 didn't have an upgrade utility, then you'll be relieve to know that 4.2 does have an upgrade tool and it should be out in the next few days!

I just checked to make sure. I'm running 4.1 SP1.

I recommend getting this called in to customer services so a case can be escalated to the developers.

Anyone else having this problem? I have a customer stating they to are seeing this.

Well, after attempts to search the data to find an errant name, as well as several other suggested approaches, I still couldn't find what was causing the issue.

Reading that it's usually a bad name, and that it is fixed in 4.2, I decided my only other option is to attempt to upgrade to 4.2 before calling in a problem. I ran the upgrade this morning, and it seems to have corrected it! All menu items are back, drill-down's are working again, and tasks are running as they are supposed to.
