system management homepage "Error Authentication failed"

When I first logged in SMH it shows message "Athentication falied" and then I clicked button "Return to System Management Homepage" and type user and password then it finally shows "loading data..." properly
why is that?
what is cause?

Which version of SMH are you using.

Can u give me more details.
