Tape I/O fails in Windows 2K with Device I/O Error

I am using an AIT Tape drive in Windows 2000 Server. While doing TapeIO, the read operation is failing with the "Extended Error: 1117 The request could not be performed because of an I/O device error."
I am using a Adaptec 39160 SCSI card to connect the tape drive. The same operation is passing with older models of Adaptec card(2940UW and 29160) in the same system and with the same drive.
The same situation exists with Windows 2000 Professional also.
Could anyone please tell me what can be the reason for Device I/O Error?
