Ultrium-1 215 External 0x45d

Recently I get following error from Veritas Back up
Final error: 0x45d - The request could not be performed because of an I/O device error.

29160LP and tape installed with latest driver and firmware. Backup runs for almost 2 hours and then error display, when I run HP diagnostic on the tape after error, it can not find the drive. if I turn it off and on then Diagnostic software will see the tape and shows no error.

you didn't say what O/S you are running. If Windows, make sure Removeable Storage service is disabled in services

It is windows server 2003 standard. Removable storage services are disabled.
