Migrate from P4300 Single Cluster to Multi site Cluster.

We have a 2 node Single Cite Cluster on P4300, SAN IQ 12.5. We are wanting to migrate to a Multisite Cluter. We are adding an additional node to the current Cluster, and have 3 more nodes to build the additional site with. Can we just change form Single to Multisite Cluster? is there a migration guide that can be used for this?

As far as I know you should be able to migrate to a MSC as long as your management group meets the requirements, e.g. same version, etc. 

I've tested this process in a lab environment and it worked but everything was running LHOS 12.6 but I know the P4000's are stuck on 12.5.

You'll need to configure your "sites" to make sure you are splitting the nodes out logically as well as having an independent FOM if you want automatic failover when losing a rack. The Multi-site clustering guide should have most of the information you need but there isn't a specific step-by-step guide that I've seen.
