9.5 is x64 bit only I believe so depending on what you are running on that can be an issue.
Side question. If it fails, why bother trouble-shooting unless you want to do it on principle.... just remove that one from the management group and install a new FOM.
FYI, I have no problem upgrading managers so it sounds like something funky with your setup, but by the time you figure out the problem, you might as well have just removed the old and made a new and be done with it.

Nice that the installer doesn't tell you that you should first install a 64bit FOM.
I'll have a look at reinstalling a FOM,

Re: Upgrade 9.0.00.3561.0 to 9.5.00.1215.0 fails
Hmm odd, before I attempted the upgrade I did remove the old 8.1 FOM and added a 9.5 FOM, but I got the message (can't remember exactly and I am out of the office) something like "cannot add a FOM of a higher version to a lower version of the management group"
So I re added the old 8.1. FOM. I did think of removing the FOM whilst the upgrade was in place but then I would loose quorum.
I have a open call with HP and will have a look when I am back in the office next week.

Re: Upgrade 9.0.00.3561.0 to 9.5.00.1215.0 fails
Ok I have one cluster (a two node with FOM) upgraded!
I couldnt tell you why it wasnt working, in the end I used a VM with the CMC installed, that was basically configured the same as my previous physical box.
The only issue I have now is that on one of my nodes the network bond too the mac address of nic2, now the bond is using nic1 mac address so my license key is invalid!
Ill email HP and try and get a new key.
Then onto my 8 node cluster in two weekends time!!!!!!!
