HP4000M trouble

We have an HP 4000 M switch, works fine, except that from time to time, it seems to work like a hub rather than a switch, that is dispacthing paquets to all ports instead of the required one. It is not only on booting, but seems to happen sometimes with no reason.
Any explanation ?
