Windows 2003 DSOD storport.sys and cqmgstor.exe
Hi All
We recently installed the HP Health Tools on a DL380 G5...
cp011797.exe - HP Management Controller Driver
cp013106.exe - HP Insight Diagnostics
cp006253.exe - HP Insight Management Agents
cp013669.exe - HP System Management Homepage
A few hours later the server suffered a blue screen of death (attached).
After using the Windows Debug Tool (windbg.exe) I can see the following which looks suspicious...
Unable to load image HpCISSs2.sys, Win32 error 0n2
*** WARNING: Unable to verify timestamp for HpCISSs2.sys
*** ERROR: Module load completed but symbols could not be loaded for HpCISSs2.sys
Probably caused by : storport.sys ( storport!StorPortGetPhysicalAddress+43 )
We also see this:
DEFAULT_BUCKET_ID: DRIVER_FAULT_SERVER_MINIDUMP
BUGCHECK_STR: 0x3B
PROCESS_NAME: cqmgstor.exe
CURRENT_IRQL: 2
LAST_CONTROL_TRANSFER: from fffffadcbf6fa656 to fffffadcbf14b743
This server was the active node in a 2 node Windows 2003 Cluster linked to an IBM DS 4800 SAN.
Is there en issue installing the HP Health Tools on the active cluster node?
The passive cluster node (now the active) seems healthy and stable.
Do you have any information on how to prevent this from happening again?

thanks for your post.
HpCISSs2.sys is related to smart array controller driver...
so I would suggest to upgrade smart array controller driver and firmware if this does not help.. then get the latest storport driver from microsoft website and update it..
also would like to check if NIC teaming is configured in your environment as I experience some issue with NCU version though it does not sound releated.

We are using an older raid controller driver so an Update would be wise.... I will need the schedule this though...
Yes, we are using NIC Teaming with a HP Onboard NIC and a HP PCI-X NIC.
