Recently moved server. Now ioscan and lvm don't match

We had to physically relocate our servers this last year.  

 

Subsequent to the move the ioscan shows that the SAN drives are c21t0d0 - c21t0d4.

 

Prior to the move the drives were c13t0d0 - c13t0d4

 

Now if I run ioscan I see c21, but if I run lvdisplay the physical volumes are listed as c13, and the sam console indicates that there is a configuration error as c13 no longer exists.  

 

Despite these discrepancies, there has been no loss in system functionality or performance.  The logical volume group is still working fine under c13.  Because of this I did not even know we had the problem until I was preparing to add a new drive from the SAN.  I need to know how to fix this.  Any help is greatly appreciated.

you can vgreduce the old pathes and vgextend the new pathes. I assume you are using different ports on the switch - hence the new device files.
