Fibre Channel Controller Status Change. The Fibre Channel Controller in Slot 1 has a new status of 6.
(Host controller status values: 1=other, 2=ok, 3=failed, 4=shutdown, 5=connectionDegraded, 6=connectionFailed)
[SNMP TRAP: 16028 in CPQFCA.MIB]
We also see the following in the system management homepage; state as degraded
Qlogic QMH2462 4GB FC HBA for HP c-Class Blade System
Status: loop failed
Location: slot 1
Serial Number: MY56502093
World Wide Port Name: 50060B00006BC5E4
World Wide Node Name: 50060B00006BC5E5
Firmware Version: 4.00.23
Option ROM Version: 1.16
Storage systems: none
Tape Controllers: none
The server is Proliant BL465c G1 and it's running on Windows Server 2003 R2 Std Edition, SP2
Is the HBA card faulty OR need to do an firmware update?

we are supporting this server remotely so physical visibility is not valid. We can see the disks from computer mgmt.
We have checked with our SAN team and they said there's nothing wrong with the physical connection between switches,fiber and SAN. connection. They said the error comes from the server itself.

If I uninstall those agents do I need reinstall the system management homepage as well?

yes you can install the newer insight management agents on top of the current one, no problem to upgrade this way.
Could you relate the date&time of the error on the fibre channel card to other events in the SAN or network ? i.e. another server who reboots.
Are there other servers/blades that also show this error and if so, do they show it around the same date&time ?
It can be that another device (server/switch etc etc) restarts and causes an short SAN reset or timeout issue between the storage array and server/blade.

Thanks for the feedback and suggestions. Well, I have investigated this issue thoroughly and found the affected server is in DMZ and SAN is not attached. I'm going to disable the HBA card in device manager so there's no more alert.
