5412zl Module DOA after Reboot
I recently upgraded all of my 5400 switches' software. I had three different switches boot up but one module in each did not start back up. One of the switches I was physically watching and when all of the other modules initialized this one did not. The switch displayed a red alarm in the upper right hand corner. I simply reseated the module and it came back to life.
The problem is one of my remote switches had this same problem but since no human was watching the switch reboot then no one noticed that switch lost a module. I have scoured through the event logs for that switch and I can see where it rebooted but I cannot find anywhere that an event was triggered for the module fault. This module has been down for several days and no one noticed but that concerns me.
I went to the remote IDF and sure enough there was the red alarm for module "G". so I logged in to the web gui and even there nothing alerting me that there was a module having a problem. It seems the only way to know this is to be physically looking at the switch.
Do the 5400 and 8200 send an event if there is a fault with one of the modules during boot? Has anyone experienced this before?

You should probably log a call for this, especially if the failure in 3 different switches was for the same module type or location within the switch.
Since the switch had the fault light on, you would expect that the logs and SNMP traps would have recorded something.
Try "Show module details" to see what is reported for module G.

I had the same issue recently
Upgraded firmware on E5412 and a module (J9534A) was dead after reboot
I have 2 more to upgrade, so I am hesitant to do it now
Additionally, only reason I knew was the Flashing Lights on the module there was nothing logged in the web interface or I could see in the event log
We also log to Syslog, so when I have a change I will look in there for the switch

Since then I make sure I compare "before and after" from the GUI. I will do a screenshot of the GUI before and then compare it after the switch comes back online. I logged a ticket with HP... but I will let everyone guess how that went... LOL
