Random port security violations

Every day we are having port security violations with the intrusion MAC being xxxxxx-000000. The intrusions happen on random ports and different blades. We though it might be a loop but dont see any loops.

It's also causing port securty violations when the ports are set to continous and violation action set to none (which makes no sense).

Any one experience this issues or may know what may be causing it?
