What final is story of this?

An error was detected on device \Device\Harddisk4 during a paging operation.

I got it 10 times per second.

It's look like you lose your volume. Any other detailes? You have redundant connection? MPIO or SecurePath? Which switch(es)? Switch and mezzanine card firmware/driver? Other devices in SAN environment?

SecurePath 4.0c Sp1, B-series 2/64, fw 4.0.2,
driver 9.0.0.13, just messanine and EVA5K 3020 in that Zone.

You can see disks and bl servers from switch? All connections are up and working (some errors on switch ports?)?
If you have correctly presented host/LUNs from EVA and configured zone, such behaviour can cause faild FC cable (I had such problem).

Other thing: HP recommend switch firmware revision 4.2.2a

Yes, some portfaults at switch end. But hard to believe that all my cables, tens of those are faulty.

Pls, use v4.2.2b there is something wrong in v4.2.2a.
