3 incident management problems

Hi people, can anyone share some experience with me. I want to know 3 administrative (not cluster or backup) strictly administrative incident management problems that u have incurred and how u solved it.

1) For two D boxes, our support costs were $29,000 a year, hardware and software. I managed to cost justify three new L class boxes based on the savings in costs during the first five years of ownership. The capital expense was literally paid for by lower support costs. Kind of hard in the end to get the users to give migrate their applications off the D boxes, but it did happen a year late in the case of the second.
