Auto Port Aggregation working with FEC

I am having 2 problems with APA. The first problem is I cannot get FEC (fast ether channel) to work. I have installed APA on a D350 with a dual-port NIC (J3516A). The switch I am using is a Cisco 6500. I have the cards and switch set to 100 Full duplex. I have checked lan1 and lan2 to make sure they are both set to FEC_AUTO. The port status says they are both in stand alone mode. When I run lanqueryconf -s I get a message that says lan1 is not configured in LAN_MONITOR mode. I don't want lan1 in LAN_MONITR mode I want it in FEC_AUTO mode. The netconf file has lan1 configured. I tried putting lan100 (Aggregate link) in the netconf, but every time the system came up the network failed.
I did get APA to work in LAN_MONITOR mode so I could use the failover part of the software. I do have a valid lanconfig.ascii file from when I had the ports in LAN_MONITOR mode. I used the lanconfig.ascii file to see if FEC would aggregate the ports, but that also did not work. I took a look at the PAgP statistics on the switch and the switch is sending PAgP packets, but they are all getting returned in error. I have looked at a lot of the APA files and cannot figure out why FEC will not work. I have also installed the latest APA (PHNE_24308) and Lan patches.

My second question is when I have the ports setup in LAN_MONITOR mode the failover from one port to another port works great. But I am having problems when I have the primary port failover to the secondary port and then the server reboots. When the server comes backup the network fails and I don't have a network connection. I have to bring the primary port backup then I can recreate the aggregation manually. Is there anyway to bring the server up on the secondary port so I have network connectivity?
