How u restore back? Are the server need to shutdown ?

disk?
Check any stale PE in pvdisplay -v
Check disk in ioscan
Check syslog for any errors
Check event logs for disk failure alert

Depends on server. If disk is hot swappable then you need not to reboot.

You could also try the follwing to check disk integrity
dd if=/dev/rdsk/c?t?d? of=/dev/null bs=1024k

see when_good_disk go bad pdf
in that pdf it is mentioned one by one
how to identify failed disk and how to remove it how to back up it in simple way manner.
someboey posted already that one so i don't want to post it again. which one is a very good reference.
