SolutionSwap space needs to be contiguous. The extension is an issue because most likely the space (on disk) "after" the swap space is already allocated somewhere else.
I would suggest making another (new) lvol for more swap. Create the new lvol (using lvcreate -r n -C y) and then use the swapon command.
Don't forget to add the new volume in fstab accordingly.

swap needs to be contiguous, but your vg00 does not have enough contiguous space to expand.
Secondary swap file can be created on a different vg.

This shows that the disk is dead.
There are two thing you should do:
1) call HP to replace the disk
2) Do some HPUX courses.
