and still get the same errors.
I have a 9000/889/K460 running HP-UX 11.00.
Here is the event log:

Description of Error:
An ioscan failed because the Fibre Channel
Mass Storage Adapter is OFFLINE.
I ran insf -e. Here is ioscan:

Have you checked your fiber cable and the port on your sab switch for errors?

To check the fiber connection. The man page is full of good stuff.
mstm cstm or xstm : Test the hardware.
insf -C disk
Will initialize disk.
