I got /dev/vg00/lvol3 mounted on / and lvdisplay -v on that LV tells me it's stale. I tried lvsync and it came back with IO error.
vg00 got 2 disks (mirrored)
lvdisplay -v shows all LEs are current except one LE (current on 1 disk and stale on the mirrored disk).
what's next ??

you may have a bad block on the disk. Do a dd on the disk on lv on which the stale extend is available and you may find out if the disk is a problem. By default i think reloaction is off for vg00/lvol3 (root).
dd if=/dev/vg00/rlvol3 of=/dev/null
you should get
some_number+0 blocks in
some_number+0 blocks out

Before replacing your disk, and if the thread that Sanjay has added does not do the job, this is an opportune time to make sure that you have that ignite tape up to date.
# make_tape_recovery -x inc_entire=vg00 -I -v -a /dev/rmt/0mn
