Right off, this typically means that you may have a disk that has failed..
You may want to start investigating all of your disks...
After an ioscan -funC disk, do a diskinfo on all of your disks to find the culprit..

First thing I would look at is at there are some SCSI errors.You need to check the device
b_dev: 1f005000
i.e.c0t5d0
check the vg to which the disk belongs.There might be some problem with that device.
2.Also there are some Hardware reset messages on the network card .Please do check the speed and duplex settings of the card and the switch are the same.
