The strange HD is c2t0d0 (disk 4). It's a hot-swap disk. When I insert this HD into the box, it shows "claimed" in "ioscan -fnCdisk", and just one or more hours later, it become "NO_HW" for "ioscan -fnCdisk".
Then I pull out this HD and insert it again, the OS shows "claimed" again. And later it become "NO_HW" for "ioscan -fnCdisk".
It seems that "vgdisplay & diskinfo" will show error message but I'm really amazing that "sar" work fine!
Any ideas?

SolutionThat disk has failed or can not be put into service.
dmesg should confirm this.
If you are running ems, it should have emailed root a warning as well.
If its a hotswap you might try removing and reinserting the drive.
You might try increasing the timeout on the device, but I pretty much think its dead. It could be the drive cage or cable, but is most likely the drive itself.
