Your system appears to have had a large problem. From the looks of it I would say that a controller card has gone out. Its possible there is an unterminated scsi device on it or one of the devices on the chain (#4) has failed.
Its also possible a cable has gone out.
This is a situation I usually leave to the good folks in hardware, though you could eyeball the situation, as in look around and try and find an obvious problem.
The issue is on controller in pci slot 4.
Also, its always helpful to post your machine type and your OS version along with some idea of patching.
I think you have enough data to file a hardware case and make this somebody elses immediate problem.
