Re: SCSI error in dmesg
I've seen similar messages in a few situations:
1) I've just removed or inserted a disk drive into a HASS unit
2) A device on the bus is failing
3) A SCSI card is failing.
4) Cabling/termination problem
Check termination and cabling on everything, and if you've got a hardware
contract with HP you might give them a call.

Re: SCSI error in dmesg
Do you have ServiceGuard?
I think the the "requested from above" means that an application requested the
reset, ServiceGuard does this when it is obtaining the cluster lock disk. If
this is the case there is no problem.

Re: SCSI error in dmesg
check syslog for the error, and in the line(s) of text see if there is a
dev=0x######. You can trace the error back to a specific device this way. Are
you running a backup at this time, or are you trying to start a backup or
restore to a tape drive that is already busy ?
