unable to recognize disk drives

I have an L-2000 running hpux 11.0 that has a 4x2 enclosure of 9gb drives of which only four show up on the "ioscan -fnC disk" command. I see device files under /dev/dsk as
c6t3do-c6t6d0 yet I still can't see them to pvcreate/vgextend..... Do I have a bad channel and if so how do I determine that. See attached full ioscan and ls of /dev/dsk. Any help from you experts will certainly be appreciated.

How about doing this:

insf -e

and then attaching the output of

ioscan -fn

and a diskinfo -v

Also, it's helpful to know which IO paths are really duplicate paths.
