I am seeing the following error on my UNIX box. How do I diagnose the problem? How do I go ahead? if you require any other information please let me know.

What OS?
What is your servers model?
What types of storage is attached?
Did this just start happening?
Did something get added to it hardware wise?
Anything change software wise?

Thanks for your responce, Nothing was added, and nothing was changed ot the Box. its Attached to SAN storage.
