First thing to do is read the upgrade guides, look under the "Installing and Updating" section here :

First of all read the "README before ..." guide and then the installation guide. If possible do a test upgrade first. I've seen/heard of a few issues, nothing too major as long as you plan it well. I'm sure others have more "real world" experience they'll share with you though.....

The upgrade installation OS requires more space on the system file systems.

SolutionIssues:
Cold install is more reliable. Rent, beg, borrow what you need to start off with cold installs. There are problems with update-ux though its getting better.
11.11 requires more memory just to run when installed on hardware that ran 11.00 easily. You need to get all the current Gold Packs in, the Bundle, the Hardware Enablement patch and on slower machines every sd-ux patch you can get your hands on. sd-ux runs like a dog on older hardware.
Oracle and many database vendors have a torturous process for upgrading the data and binaries from 32 to 64 bit. Its best and sometimes required to upgrade versions in place, migrate the data and then upgrade the database to the latest version. To try and do everything at once leads to a lot of lost sleep.
Most applications will run on 64 bit HP-UX 11.11 in 32 bit mode if they can't be compiled 64 bit. There are exceptions. Every minute bit of software needs to be checked for compatiability.
Leave yourself enough time, have a good project plan with test plan steps. Make lots of make_tape_recovery backups along the way.
