Warning: unable to read a volume header

Hi, I need help with regards to creating volume headers. I get this type of message whenever I backup using "fbackup" utility.

You'll get this message when you are attempting to write to a tape that is not already an 'fbackup' tape. 'fbackup' places it's own header on the tape. If it was used before it would say fbackup tape and I have been used xxx amount of times. If this is not the case, please advise the exact 'fbackup' error number.
Anyone for a Mutiny ?
