"Permanent IOA failure"

Hi,
I have new IBM p-550 server on which we installed RHEL AS4 power series using HMC.

first i was able to detect the external array and configure the RAID 5 on it,

but due to some reasons, i needed to re-install the OS,

I re-installed the OS, but now i am not able to detect the external array,

at the boot time, i use to get this error message:

"Permanent IOA failure"

Please suggest.

Seems like You have to replace broken RAID disk controller PCI-X adapter.
