Is DTSS enabled on the system? This may have been configured with DECnet-V. DTSS is another time service available which may be causeing problems. I haven't used DECnet V or DTSS so this isn't from "live" experience.
Try:
$ show logical dtss$_service_active
If the logical is defined:
$mcr ncl
ncl> show dtss all
ncl> disable dtss
ncl> delete dtss
To make sure it's gone, type the following commands:
ncl> show dtss all
ncl> exit
$ show logical dtss$_service_active
You can put a "$ EXIT" at the start of SYS$STARTUP:DTSS$STARTUP.COM to make sure it won't restart on the next boot.

The so far provided info suggests to me that this is an fairly modern alpha (at least it does have apparently either four DE600 or maybe more likely two DE602 nics). VMS also would at least V7.1-2 and you do have DECnet Phase IV stuff instead of DECnet/OSI (DECnet/plus) so the DTSS stuff is not involved.
I would also expect TCPIP rather than old UCX being in use.
And probably the NTP service definition, TCPIP$NTP account, SYS$SPECIFIC:[000000]TCPIP$NTP.DIR, SYS$SPECIFIC:[TCPIP$NTP]*.*
are fine. So the remaining thing to check would be system wide login prodecure, i.e. the one pointed by logical name SYS$SYLOGIN, typically SYS$MANAGER:SYLOGIN.COM.
Edit that file and have it started like
$ set noon
$ if f$edit(f$getjpi("","USERNAME"),"COLLAPSE") .eqs. "TCPIP$NTP" then exit
After that restart ntp service using
$ @sys$startup:tcpip$ntp_shutdown
$ @sys$startup:tcpip$ntp_startup
I would expect that the SYLOGIN procedure contained some strange stuff causing havoc to
non-interactive logins and above edits will effectively bypass that stuff for the NTP server process.

if you have an ES47, ES80 or GS1280, this is a known problem:

Please obtain and install TCPIP V5.3 ECO 2 or higher. Most recent patch is: TCPIP V5.3 ECO 4.
Regarding the repeating DECnet Phase IV Line synchronization lost messages on EIA-2 and EIA-3, I bet that you don't have any network cable connected to those 2 ports.
If true, just consider to delete/disable the DECnet lines/circuits EIA-2 and EIA-3:
$ MC NCP
NCP> SET CIRC EIA-2 STAT OFF
NCP> SET CIRC EIA-3 STAT OFF
This will stop the DECnet circuit fault messages showing up every 10 seconds. Please consider to also set up a new version of SYS$MANAGER:OPERATOR.LOG ( $ REPLY/LOG ) and PURGE the old one, which may have reached considerable size due to those repeating DECnet messages !
If there is no cable connected, DECnet will get a send failure, whenever it tries to send a packet to that network device(s), this causes the circuit fault.
NCP> PURGE CIRCUIT EIA-2 ALL
NCP> PURGE CIRCUIT EIA-3 ALL
NCP> PURGE LINE EIA-2 ALL
NCP> PURGE LINE EIA-3 ALL
will get rid of those un-connected DECnet circuits/lines in the permanent DECnet database, so those won't show up again after the next boot.

according to your PROD SHO HIST output, you have TCPIP V5.3 ECO 2 installed since 23-JAN-2005.
What type of system are you seeing this problem on ? Would installing the most recent TCPIP V5.3 ECO 4 be a possibilty ?

so this matches the problem description in the ES47,... release notes - except you seem to have TCPIP V5.3 ECO 2 already installed. The information in those release notes is questionable !
I've seen this problem (just checked my old notes) on an ES80 (also with TCPIP V5.3-182 = ECO 2 installed !) and it has definitely been fixed by installing TCPIP V5.3 ECO 4.
Here is the release note from TCPIP V5.3 ECO 3:
ECO D 14-JUL-2003 Alpha only
Problem:
NTP fails to start after error message "VMS timekeeping is not working as expected" for ES47 Alpha machine.
Deliverables:
TCPIP$NTP.EXE V5.3-18D
Reference:

You could check your version of TCPIP$NTP.EXE with: $ TCPIP SHOW VERS/ALL
If you're not at V5.3-18D or higher, you'll have to install TCPIP V5.3 ECO 4 if you want to successfully run NTP.

I'm with Volker. You have to install tcp/ip eco4 if you want to start ntp service. There is no alternative.
Looking at your configuration, I saw you have DecNet IV, so I understand why your circuit EIA-2 and EIA-3 can't start. DecNet phase IV is able to work with only one NIC. Therefore you have to disable (just for DecNet) circuit EIA-2 and EIA-3.
Execute follow steps:
$ MC NCP
NCP> SET CIRC EIA-2 STAT OFF
NCP> SET CIRC EIA-3 STAT OFF
NCP> DEF CIRC EIA-2 STAT OFF
NCP> DEF CIRC EIA-3 STAT OFF
The above SET commands stop messages immedialty and DEF stop definitevly even if you boot.

did you read the release notes I pointed you to ? This BADLOGIC problem in TCPIP$NTP only affects EV7 systems (ES47, ES80, GS1280). An ES40 ist NOT an EV7 system, that's why TCPIP$NTP;1 V5.3-18B works on that node.
Sorry, but installation of TCPIP V5.3 ECO 4 is the ONLY solution to this problem.
