SWXCR Hard Error Detected

Hi,

I've AlphaServer 4100 running Digital Unix 4.0D with following patchsets installed:
- duv40das0009-20010724
- duv40db17-c0061401-12858-e-20020115

System installed on external Raid 230/Plus w/ 4x36GB disks all in JBOD mode.

System disk splited into 2 partiotions, both uses AdvFS filesystem as follows:
root_domain#root /
usr_domain#usr /usr

/var partioion linked to /usr/var


Today at morning I found that server freeze and console shows black screen. After reboot in single user mode I tried to mount /usr partition and get following error:

then server crashed and rebooted again.

I tried to /sbin/advfs/verify but it printed out same error and reboots.

Can I repair my /usr partition or I should install OS from scratch?

The panic indicates a metadata corruption within ADVFS. If swxcr logs an error it seems to be hardware related. Please check hardware first, fix it and recreate the domain.
