Proliant ML370 G2, NetWare 6.5 and Insight Agents 7.90
This is running Native NetWare (not XEN virtualised).
Has anybody else had this problem?

yes, my customer experienced the exact same Problem on a HP Proliant DL 380 G3.
Problem is in the module cpqhthsa.nlm as you stated before.
Would be nice if Hp fixed this, as the machines are shown in critical status in SIM.
Maybe it's marketing, so that customers buy more fans...

Server Running NetWare 6.5 and the HP ProLiant Support Pack (PSP) for NetWare Version 7.80, 7.90, or 7.91
On an HP ProLiant ML370 G3 or ProLiant ML370 G4 server running NetWare 6.5 and the HP ProLiant Support Pack (PSP) for NetWare Version 7.80, 7.90, or 7.91, the HP System Management Homepage (SMH) may erroneously list Fan #2 (CPU), Fan #4 (CPU), and Fan #6 (I/O Board) as absent and failed in the Environment Recovery section of the SMH, under Overall Fan and Thermal Status.
Note: This only occurs in server configurations in which these fans would be required solely for redundant operation.
Any HP ProLiant ML370 G3 or ProLiant ML370 G4 server running NetWare 6.5 and the HP ProLiant Support Pack (PSP) for NetWare Version 7.80, 7.90, or 7.91.
These reported fan failures, which are specific to redundant fans only, are erroneous and can be safely ignored.
For the ProLiant ML370 G4 server, the erroneous fan failure reports will be corrected in a future version of the PSP. When more information becomes available, this advisory will be updated.
However, for the ProLiant ML370 G3, the erroneous fan failure reports will not be corrected because this server is retired and no longer supported.
